Contact the proper authorities
When a death occurs, a legal pronouncement of death must be declared. If your loved one dies inside of a medical or nursing facility like a hospital or nursing home, the staff will handle getting this declaration made, typically by a doctor or registered nurse. If your loved one dies at home and not under hospice care, you need to immediately contact local emergency officials to come (dial 911); police, medical and fire may respond and will request a coroner to come. Typically a coroner will come to pronounce the death. When the loved one was under hospice care while in home, you should contact the hospice agency to come and a registered nurse comes to make the declaration.

Information Required:
- Full name and address
- Marital status
- Race/Ethnicity
- Date and city of birth
- Highest level of education
- Father’s name, mother’s maiden name
- Name of spouse
- Occupation and industry, even if retired
Extra documentation we may need
Our staff may need important documents to complete any legal paperwork. These documents include:
- Drivers license or ID card for proof of name
- Social Security Card or statement from Social Security or a filed tax statement showing the number
- Birth certificate, showing parents' names including mother's maiden name
- Military discharge documents, like a DD214
- Life insurance policies when desiring to use a policy on an assignment basis to pay for services
